Requesting APPR Scores

1. Parent/guardian will fill out the form: “APPR School District Request Form 2017.”
2. Forms should be returned to the District Administrative Center, 6 Bluebird Road, South Glens Falls, NY 12803 by mail or in person.
3. The district will verify that the information being requested is within the law. (Request must be made by a parent or guardian for a student in the current class or building of the teacher(s)/principal.)
4. A representative from the district office will call you to set up an appointment with the superintendent or assistant superintendent where you will receive the score.
5. You must bring photo identification to the appointment so the district can verify your identity.
6. Once the meeting is complete, the teacher or principal will be notified that their score was given out.
